We are seeking a driver for our Medical Case Management Program. Agency vehicles will be provided.
JOB SUMMARY:
The Driver/Transportation Aide works with the HAP Medical Case Management (MCM)Team to provide door to door medical transportation services to clients in HAP. The transportation may be provided by the Driver/Transportation Aide or scheduled with a contracted provider.
The Driver/Transportation Aide will interface with Medical Case Managers to discuss Transportation and Logistics regarding specific transportation needs of clients. Some examples of rides to be provided include (but are not limited to) medical care appointments, labs, dental care, mental health treatment, pharmacy, medical case management appointments, and support group meetings among others. The Driver/Transportation Aide is expected to demonstrate a professional and respectful demeanor in dealing when interactingwith clients while maintaining their privacy and confidentiality at all times. given the sensitive nature of this work.
RESPONSIBILITIES:
1. Completes required Defensive Driving course, and completes training on Pennsylvanias Act 148 (Confidentiality of HIV-Related Information Act), Relias Introduction to HIV/AIDS, Universal Precautions, Preventative Transmission of Tuberculosis Exposure, agency drug and alcohol policies, and agency policy and procedures.
2. Utilizes close supervision for quality assurance and adheres to agency policies, procedures, and practices while carrying out direct service duties promptly and accurately.
3. Accepts requests for transportation from MCMs and schedules transportation for clients in a timely manner, including rides provided by the Driver/Transportation Aide, Lyft, Bucks County Transport (BCT), and any other contracted providers.
4. Completes documentation for all scheduled transportation, and notifies MCMs of successful scheduling and final status of each request.
5. Assists with monitoring of fund availability for Lyft, and assists with preparing check requests for BCT.
6. Completes and maintains orderly time sheets and expense sheets and submits them to the Transportation Scheduler HAPC in a timely manner as required by the agency or its contracts.
7. Understands and observes each clients rights to privacy and confidentiality as articulated by agency policy and procedures.
8. The Driver/Transportation Aide must have a current and valid State operators license and insurance.
9. Uses supervision to identify and address professional development needs. Specifically
tends to on-going development of sensitivity to diverse cultural groups, particularly
groups served by this position.
QUALIFICATIONS:
Education
High School Diploma or equivalency such as GED.
Experience
Pre-requisite job skills and aptitude for on-the-job training may be demonstrated by documentation of successful prior employment, volunteer or relevant life experiences.
3. Other Credentials
The Driver/Transportation Aide must have a current and valid drivers license and auto insurance. The agency may require specific credentials or documented training/experience for specific assignments covered by this Job Classification and Title.
The letter of hiring or as appendix to this Position Description will detail such credentials, e.g., training/experience; operators license, current and valid; ownership or use of an insured automobile; understanding of agencys confidentiality policy; health certification; police clearance for working with children; etc.
