CASE MANAGER- COSTAR Homeless Program-South Jersey Behavioral Health Resources
SOUTH JERSEY BEHAVIORAL HEALTH RESOURCES, INC. (SJBHR), a 501(c) (3) which may qualify for Public Service Loan Forgiveness. Is a community-based mental health organization dedicated to providing quality behavioral health services since 1984. Salary is $19.34 per hour, with increase to $20.34 per hour effective July 1, 2023. We offer a robust benefits and time off package.
The COSTAR Program of SJBHR is currently seeking a seasoned, motivated individual to serve as a full-time case manager. The individual will work with a small team providing services to homeless persons with mental illnesses both on-site in our clinical setting in Camden, New Jersey, and off-site in the field at congregate sites with the targeted population.
Responsibilities:
Under the supervision of the Program Manager or designee, the case manager will provide information, referrals and linkages to consumers such as temporary and permanent housing, mental health services, medical and dental services, drug and alcohol programs as well as habilitation and rehabilitation services, The case manager will be responsible for both hard copy charts and use of cloud-based (HMIS), as well as monitoring a day drop-in center.
Requirements:

Graduation from an accredited college/university with a Bachelors degree in psychology, or a related field and at least one (1) year experience in the provision of services to the target population is preferred; or
A combination of one or more years of college, plus one or more years related experience together equaling five years; or

Valid drivers license is required. Drivers abstract must meet insurance carriers requirements. Valid CPR and First Aid certification is required upon hire.
