Join our SELF (Seniors Empowered for Life Fulfillment) program as a Pennsylvania Certified Peer Specialist and help support older adults within the Bucks County community! The SELF program provides education, case management, medication management, peer support and therapy for adults 60 years and older. This is a part-time role.
What Youll Offer:

Pennsylvania Approved Peer Specialist Certification
Valid Pennsylvania Drivers License
High School Diploma or GED
Ability to perform the physical requirement of the job description, with or without accommodation
LVF requires all Associates to be fully vaccinated upon hire. LVF will consider accommodations for medical and strongly held religious beliefs

What Youll Do:

Provide peer support to adults who are qualified for and accepting of this service in their home and community
Document and file (in chart) assessments, individual recovery plans, progress notes, and service completion plans
Maintain peer support certification through continuing education
Regularly attend the Professional Development Network for certified peer specialists working in Bucks County
Regularly attend Peer Team Meetings and SELF Team Meetings
Role model and discuss the value of peer support in the recovery process
Participate in LVF recovery initiatives and events.
