Job Highlights

Registered Nurse

The Registered Nurse (RN) provides intake and assessments for consumers and families. Collaborates with the treatment team to develop and implement individualized recovery plans and monitor progress toward wellness and recovery. Provides education and consultation on medication and health maintenance. Evaluates levels of service intensity required to ensure continuum of care.

Salary Range: $75,000-$85,000

LPN Manager

The LPN Manager is the Team Leader for the residential group home working closely with the Registered Nurse and the Residential Team. The LPN Manager coordinates activities to ensure seamless program operations, covering 24 hours per day and supported by the Residential Nursing Assistant carrying out the day-to-day program activities with the Team.

Salary Range: $24-$26

Residential Counselor/Certified Nursing Assistant

The Residential Counselor/Certified Nursing Assistant, works closely with the LPN Manager, and at times, covers in his/her absence, to provide direct care support for consumers residing in a 24 hour supervised residence. Coordinates activities and groups to ensure seamless program operations that promote wellness and recovery initiatives for consumers. Oversees coordination of group activities and ensure compliance with regulatory bodies, and agency policy and procedures. Assists consumers in establishing goals and monitoring progress, to ensure continuity of care and progression of consumer’s independence.

  • CNA CERTIFICATION PREFERRED, or willing to engage in coursework to obtain the CNA designation as supported by the agency.

Salary Range: $18.50-$20.00

Community Support Specialist

The Community Support Specialist (CSS) is responsible for providing supportive services in the community and provide in-home support and skill development to improve individuals’ overall wellness. The CSS promotes psychiatric rehabilitation principles and assists individuals with maintaining housing stability, actively engage community integration, self-advocacy, financial literacy and self-sufficiency. Works closely with treatment team to ensure continuity of care and working toward achievement of recovery and goal plans.

Salary Range: $18.00-$20.27

We are looking for self-starters who are willing to collaborate with nursing homes to transition individuals into a newly renovated home.

You will work as part of the Team that provides case management, linkages and behavioral tailoring to support individuals’ reintegration into the community. Build connections with their families and be a role model and life skills advocate to ensure all needs are met.

On the job training will be provided and additional certifications are available to fulfill job requirements.

Commute is close to I-195, NJ Turnpike and Route 130.
