If YES, we have a great opportunity for a Case Manager in our Supportive Housing area who is passionate, kind, and has an interest in working with individuals with Mental Illness!



The Case Manager will provide direct care services in a community setting for individuals with Mental Health needs.



This positions primary role is to use rehabilitation values, principles, and techniques to help our program participants overcome barriers to independent living.



What will you do?


Key responsibilities include, but not limited to:


Coordinate and manage the care of consumers in community support services.
Provides ongoing education and support to consumers in community support services.
Assists the consumer in sustaining wellness and recovery beyond program enrollment and achieving a degree of stability and recovery over a long period of time.
Contributes to the implementation of new ideas and knowledge that improves services and operations.
Consults with primary care physician and mental health treatment staff to support and promote successful community support services and care to achieve a high level of consumer and customer satisfaction.
Provides ongoing education and support to consumers.
Consults with primary care physician and mental health treatment staff as required to support and promote successful services and care.
Provides access to health education programs for consumers, staff, and family members.
Adheres to compliance with licensing, regulatory, accreditation, and internal standards through inspections, peer audits, audits of clinical and administrative procedures, and documentation.
Contributes to the development, implementation, monitoring, and updating of the individual rehabilitation plan and individual rehabilitation plan review.



QUALIFICATIONS AND EXPERIENCE:


Bachelors Degree in Social Work, Psychology, or a related field from an accredited college or university.
Working knowledge of computer software including word processing and excel.
Possess a valid drivers license and an acceptable driving record.
Excellent verbal and written communication utilizing the English language.
Planning and organizational skills.
Ability to perform moderate mathematical functions.
Ability to effectively interact with all levels of personnel and the general public.
Ability to travel to a variety of geographical areas where community support services are offered within the agency.
