Description:
St. Mary Home Care has a need for a Full-time Occupational Therapist. The Occupational Therapist consults, evaluates, plans and administers skilled Occupational Therapy services prescribed by a physician to homebound patients to restore function, relieve pain and prevent disability following illness, disease, injury, or surgery. Collaborates with Inter-disciplinary team members to assure patient's needs are met, and quality of care is achieved. When acting as the patient care manager, will manage, oversee and provide primary patient care delivery to a select group of patients supporting the patient centered care model, assuring quality and maintaining open communication. Interacts with any and all members of the care group both internal and external to the organization as needed.
REQUIREMENTS
Current PA licensure in Occupational Therapy.
CPR certification required.
Bachelor or Master Degree preferred.
One year clinical experience and recent home care experience preferred.

Minimum qualifications

Graduate of an approved OT educational program
Licensure as an OT in the state of PA
One (1) year experience as a professional therapist with Home care experience a plus
Must have current Drivers license and reliable transportation
