Rutgers University Behavioral Health Care (UBHC), established in 1971, offers a full continuum of evidence based behavioral health and addiction services for children, adolescents, adults, and seniors throughout New Jersey. UBHCs 1,060 experienced behavioral health professionals and support staff are dedicated to treatment, prevention, and education. UBHC, one of the largest providers of behavioral health care in the country, has a budget of $260 million and has 15 sites throughout New Jersey. Services are readily accessible and include: inpatient, outpatient, partial hospitalization, screening, crisis stabilization, family/caregiver support, community outreach and case management, supportive housing, supported employment, prevention and consultation, employee assistance programs, and a licensed therapeutic school from preschool through high school. Specialty services include the New Jersey suicide prevention helpline and peer help lines for police, veterans, active military, teachers, mothers of special needs children and child protective service workers. In FY2016, UBHC treated 16,199consumers, had 24,502 admissions, and touched the lives of 19,441 individual callers through peer support. In addition, UBHC is the primary mental health training resource for the New Jersey Departments of Human Services, Children and Families, and Corrections, delivering 16,000 trainings each year.

Rutgers The State University of New Jersey is seeking a Health Care Case Manager for the Department of Child Health Program at Rutgers School of Nursing.
The Heath Care Case Management position (HCCM) functions to support the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care and protection needs of children in out-of-home placement, in collaboration with other staff of the Child Health Unit in that local office. Provides nursing case management services in home, local office and health care settings. Nursing case management services include nursing assessment of a childs health status and care needs, development of a health plan, maintenance of a health passport/health summary, ongoing monitoring, coordination of care, advocacy, and health education to assure that each child in out-of-home placement receives appropriate health care per EPSDT and American Academy of Pediatrics guidelines.
Among the key duties of the position are the following:
Upon entry into out-of-home placement, assists with enrollment of a child in a managed care entity and identification of a medical home (primary care provider), and informs the child, caregiver and DCF/CP&P, as needed.
Assesses a childs heath care status and needs by retrieving a complete health history through obtaining and reviewing all medical records and interviewing past providers and caregivers; forwards all information to the medical home and other health providers as needed.
Schedules an Initial Physical Health Evaluation, in consultation with the caregiver and DCF/CP&P, reviews the preliminary and final reports, and ensures that all recommendations for health care are completed.
Develops a child health care plan in collaboration with the childs medical home, as well as involved specialists, caregivers, other community service providers, and when appropriate, DCF/CP&P.
Facilitates transfer of care and records to an identified medical home and HMO care manager, if applicable, when the child is changing placement or upon reunification/permanent placement.
Conducts nursing placement assessments on children entering or changing out-of-home placement, when appropriate.
Provides assistance when accidents or illness occur in workplace emergencies; performs CPR as indicated, and notifies 911 and supervisor.

Minimum Education and Experience
A Bachelors Degree in Nursing or a BA / BS degree in a health related field required; may substitute years of pediatric nursing experience for degree.
A minimum of 1-2 years of experience in pediatric nursing required.
Certifications/Licenses
Current licensure as a Registered Nurse, required.
Current certification in American Heart Association Basic Life Support for Healthcare Providers required.
Vehicle and a valid license required for off-site visits.
