TITLE: CHHA (Certified Home Health Aide)REPORTS TO: Clinical Director or Designee FLSA: Non-ExemptSupervised by RN (Registered Nurse)JOB SUMMARYProvide personal care and related assigned services to hospice patients in their residence of care, which includes home, assisted living facility, long term care facility, skilled nursing facility, and hospital. Care provided is in accordance with an established plan of care and under the supervision of a registered nurse. Actively participate as a member of the interdisciplinary team. Directly involved with caring for patients and promoting comfort on a daily basis. Observe the patient, report observations and document observations and care performed. Provide timely documentation of patient services based on plan of care. Adhere to Company policies and procedures and regulatory compliance requirements and directives. Travel as necessary.QUALIFICATIONS1. Minimum age requirement of 18.2. Proof of state certification, if applicable.3. Minimum of 1 year of direct patient care experience in an institutional or home care setting and community health/hospice experience preferred.4. Compliance with accepted professional standards and practices.5. Understanding of hospice philosophy and the needs of a hospice patient and caregiver.6. Physical exam in accordance with state law.7. Valid drivers license with maintenance of good driving record and an automobile that is insured in accordance with state and/or Vitality Hospice and Palliative Care requirements and is in good working order.8. Has presented a pre-employment health certification including a negative TB skin test and/orCXR and other tests as required by Vitality Hospice and Palliative Care policy and/or state law.9. Satisfactory references from employers and/or professional peers.10. Demonstrate excellent observation, good judgment and communication skills. Ability to provide good written documentation in a timely manner.11. Ability to read and comprehend simple instructions, write simple correspondence, and effectively present information in one-on-one and small group situations to patients/families, other caregivers and members of the interdisciplinary team.12. Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.13. Flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team.RESPONSIBILITIES1. Work within the scope of the position, in coordination with management, to meet the needs of our patients, families and professional colleagues.2. Ensure quality and safe delivery of hospice services to patients/families/caregivers.3. Perform or assist patient with personal care activities as written in the care plan, including but not limited to hygiene, mobility and elimination. If unable to perform a certain task, report to the hospice RN and caregiver.4. Provide and maintain safe environment for patient. Use equipment safely and properly(footstools, side rails, Oxygen, etc.).5. Practice standardl precautions in all contacts with patients.6. Perform or assist patient/family with home maintenance activities as prescribed in the care plan including but not limited to light housekeeping and meal preparation.7. Perform support activities as prescribed in the care plan including but not limited to supporting independence, providing companionship, and providing respite for families.8. Respect the autonomy and right to confidentiality of all patients and families.9. Monitor adequate fluid and nutrition intake, temperature, pulse, respiration, skin and basic elements of body functioning and changes in body functioning and record/report appropriately.10. Practice safe transfer, ambulation, normal range of motion and positioning techniques.11. Identify need for intervention by other members of the interdisciplinary team on an ongoing basis and report to hospice RN.12. Provide input into interdisciplinary evaluation of effectiveness of care.13. Document care provided and response of patient/family. Submit documentation in a timely manner.14. Assist patient/family/caregiver and other team members in providing continuity of care.15. Work in cooperation with family/caregiver/and interdisciplinary team members to meet the emotional needs of the patient/family/caregiver.16. Perform within the limits of preparation, experience and scope.17. Assure for compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.18. Manage a caseload and deliver care in a cost-effective manner.19. Participate in evening/weekend on-call or overtime as required, conducting on-call services in a competent and responsive manner.20. Use supplies and equipment effectively and efficiently.21. Complete a minimum of 12 hours of in-services per year.22. Actively participate in team meetings and contribute positively to patient care planning.23. Regular attendance is to be maintained.24. Adherence to a code of conduct conducive with Vitality Hospice and Palliative Care policy is expected.25. Meet or exceed delivery of Company Service Standards in a consistent fashion.26. Interact with all staff in a positive and motivational fashion supporting the Companys mission.27. Conduct all business activities in a professional and ethical manner.28. The incumbents will be requested to perform job- related tasks other than those stated in this description.JOB LIMITATIONSThe Certified Home Health Aide will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the Home Health Aide will not administer medications, take physicians orders or perform procedures requiring the training, knowledge and skill of a licensed nurse, including, but not limited to: change sterile dressings, irrigate body cavities, irrigate a colostomy or wound, perform a gastric lavage or gavage, catheterize a patient, apply heat by any method, care for a tracheotomy tube, any personal health service which has not been included in the patient care plan by theVitality Hospice and Palliative Care Case Manager.WORKING CONDITIONSGeneral office, extended care facility, patient home setting, exposure to infectious diseases, travel by automobile.PHYSICAL DEMANDSRequires physical exertion. Duties of patient care during transfer activities (adjusting the patient in bed, transferring the patient from bed to wheelchair or vice versa, and chair to toilet, etc.) require that the employee have the physical ability to maneuver patients according to the care required. Additional physical abilities required during these activities will vary according to the level of patient cooperativeness, patient weight, available space and available equipment. Regularly required to use hands, talk or hear. Specific vision abilities include close, peripheral, depth perception, and ability to adjust focus.
